Effect of Nasogastric Tubes on Eustachian Tube Function and Middle Ear Pressures


Abstract
Eustachian tube dysfunction may lead to negative middle ear pressure and fluid accumulation in the middle ear. These may occur with mucosal edema at the Eustachian tube orifice. Constant irritatin of the nasopharyngeal mucosa may occur during swallowing when a nasogastric tube is inserted.
The purpose of this study was to evaluate the effects of nasogastric tubes on Eustachian tube and middle ear pressure.
3groups were divided. Group 1 was 9 patients undergoing head and neck operations whose postoperative care involoved the use of a nasogastric tube for feeding. Nasogastric tube was removed at 10th days. Group 2 was 19 patients who had medical or surgical problems and nasogastric tube lasted 3weeks. Tympanogram was evaluated at 1st, 2nd, 7th, 14th and 21st day. Group 3 was 19 patients who were unconscious and had used a nasogastric tube from 2months to llmonths.
1) In ipsiiateral ear of group 1, tympanogram showed 44% of type C1, 11% of type C2, 22% of type B and all were recovered after removal of tube. In group B, tympanogram showed 5% of type C1, 16% of type B. In group 3, 37% showed type B with effusion or adhesion of tympanic membrane.
2) In contralateral ear of group 1, tympanogram showed 55% of type C1, 11% of type C2, 11% of type B and all were recovered after removal of tube. In group 2, type C1was 5% and type B was 21%. In group 3, type C1 was 5% and type B was 32%.
In conclusion, nasogastric tubes were associated with reduced peak middle ear pressure and middle ear effusion which were. reversible in short term insertion, but long term insertion more than 2 months may induce serious middle ear problems, such as otitis media with effusion and adhesive otitis media. There was no difference between the ipsilateral and contralateral ears.
